Did a SHARPS survey and went over the whole case plan and summarized everything we have done so far with the group. Also decided on a ZOOM meeting time for Monday.

This week, our team continued making progress on our patient’s care plan by focusing on medication access and affordability. We researched GoodRx, a free platform that helps patients compare prescription prices and find coupons to reduce medication costs. We discussed how this tool can be used to support our patients’ medication adherence and financial well-being.

Overall, our care plan is moving forward steadily. We are currently reviewing community resources and ensuring that our patient understands how to use the GoodRx app effectively. One minor roadblock we’ve encountered is coordinating communication between team members’ schedules, but we are resolving this through shared updates and planning ahead for meetings.

A key new resource we learned about this week was GoodRx, which none of us had previously used in a professional capacity. We believe this will be a valuable long-term resource for both this patient and future clients facing medication cost challenges.
