Summary of program: Department of County Human Services (DCHS) understands and values students and individuals who have a desire to learn, serve and provide valuable services to the community in which we live. DCHS provides opportunities that engage bright and caring people by partnering with schools, community organizations and associations with the vision of building a better tomorrow. Specific duties or major tasks: The volunteer CM2 provides assistance to Case Managers and Case Manager Assistant who serve the aging, people with disabilities and veteran populations in Multnomah County. Functions performed by the CM2 Volunteer enhance the state, federal, and local programs administered by the Case Managers as well as connecting clients with additional programs and community resources. Screening, Eligibility Determination and Client Support Services: Assist clients in completion of necessary forms. Assist clients with obtaining needed household items through community resources. In response to clients' unmet needs, make referrals to the ADVSD Helpline and community programs. Review vouchers and capture indirect contacts. Assist Case Manager with placement, by calling perspective providers or facilities. Send information to the prospective providers or facilities about clients needs. Help with coordination of moving clients. Administrative Duties: Photocopy, fax, mail, and sort documents. Pull various reports and process or distribute to Case Managers. Process returned mail in accordance with current state, federal and local rules, standards and guidelines. Assist in the redistribution of caseloads and identification of cases that can be transferred to other areas. Move cse files between Case Managers cubical area. Create/maintain documents and spreadsheets. Back-up business services functions. Maintain files for CM's by ensuring all required forms are in the file and narrated. Send missing forms to appropriate client/rep/POA of facility. Pack file for EDMS scanning Workload and Self Management: Makes the best use of available time and resources to effectively manage tasks and meet productivity expectations. Perform job duties independently and participate in teams. Practice effective self management of workload and stress. Skill Development and Professionalism: Provide consultation to co-workers of any classification and to staff of partner programs/agencies. Update case information using all appropriate computer systems. Narrate in accordance with ADVSD and APD standards and guidelines. Keep current on all policies and procedures for the programs administered. Continually improve professional skills, knowledge, and ability in order to achieve expert status. Promote a culture of respect, inclusiveness, and appreciation of diverse perspectives, backgrounds, and values. Be responsible for ethical practices at all times. Observe client confidentiality Qualifications: (list knowledge, skills, abilities needed for the job). • Work that includes a broad range of tasks and performed under general supervision or direction. The employee is fully trained and can perform the essential functions of the job independently. • Typically performs some of the duties of a professional in a support role, which usually requires less formal training and/or experience normally required for the professional level. Usually requires an Associate degree, or business/vocational training. • Problems are typically recurring and routine and are solved using established practices, policies, procedures and instructions with little or no option to vary procedures or process. • Communicate sensitive and/or confidential information (Such as HIPAA, PHI, PII, collective bargaining, etc.). • Normal sitting, standing and/or walking. May require ability to do some bending and light lifting. • Highschool diploma or GED Conditions of the job: Balancing, bending, climbing stairs, crawling, crouching, kneeling, squatting, lifting 10 – 30 pounds, operate a motorized vehicle, stooping, walking, vision,
